Middle Park Health has been proudly serving the community since 1933. We take a holistic approach to care by providing a range of health services to patients. Our primary services include emergency, primary care, rehabilitation, imaging, surgery, and orthopedics.
- Emergency: 24/7 emergency room care in Fraser, Granby, and Kremmling.
- Primary Care: We provide comprehensive medical services for individuals and families.
- Rehabilitation: We offer expert rehabilitation services, including physical, occupational, and speech therapy.
- Imaging: Some of our imaging services include X-rays, CT scans, and MRIs, mammograms and ultrasound.
- Surgery: We offer advanced surgical services with General and Orthopedic surgery.

Our History
Dr. Archer Sudan established Kremmling Memorial Hospital in 1933, then known as Middle Park Hospital. In 1944, Dr. Ernest Ceriani partnered with Dr. Sudan to establish the Middle Park Medical Association. Rapid growth ensued in the decades that followed, to where we are today, operating as Middle Park Health (MPH), with seven locations, and offering a full-service hospital with multiple medical centers that serve the surrounding communities.
MPH-Kremmling is a designated a Critical Access Hospital and boasts the first Level IV trauma center in the state. In 2012, as the population rapidly increased, our publicly elected board of trustees approved the construction of a second facility, Middle Park Health, in Granby, and contracted with the North Park Medical Center in Walden to offer services. Middle Park Health is proud to reflect a growing array of services for our multi-county communities.
Throughout it all, we’ve stayed true to Drs. Sudan and Ceriani’s vision to provide stellar healthcare to our local communities. It’s one we are proud to uphold, supporting and encouraging our community’s physical, emotional, and spiritual health. Above all, we practice medicine with PRIDE –passion, respect, integrity, dedication, and excellence.
Community & Future Initiatives
At Middle Park Health, we actively participate in our community, whether it be hosting food drives or providing funding for staff education, equipment, and community health and wellness programs through the Middle Park Medical Foundation.
We continue to forge strong partnerships and integrate advanced technology to ensure our patients receive robust care. Middle Park Health has invested in a new mammography machine to help in the battle against breast cancer and innovative ultrasound technology.
Middle Park Health’s emergency departments in Granby and Kremming have joined 16 percent of emergency departments statewide to earn the Colorado Pediatric Preparedness for the Emergency Room Certificate.
We’ve also partnered with Steamboat Orthopaedic and Spine Institute (SOSI) for rural Colorado. This partnership expands orthopaedic services in the Grand County community and enhances our ability to deliver high-quality care for orthopaedic-related injuries.
