Transparency, Chargemaster Pricing and Shoppable Services
You may use the links below to access Middle Park Health’s current standard of charges for items and services it offers, including gross charges, cash prices for self-pay and uninsured patients, payor specific contract rates, and de-identified minimum and maximum contract charges for each item or service. You may also obtain cost estimates using our price estimator tool. This information is accessible to you free of charge, and you do not have to establish a user account or password to access it.
If you are uninsured or need financial assistance, please contact our Patient Financial Navigator at 970-724-3151 to discuss how we can assist you. We have several different options that we can provide to those who qualify.
If you are insured, we encourage you to consult with your health insurance company to understand the amount you may owe. We also would encourage you to use the patient estimate tool found under MyChart. You may also contact our Scheduling department at 970-887-5886 for an estimate of your costs and out-of-pocket responsibility.
