We want you to know that we continue to do everything we can to keep our patients, staff, and visitors safe. We have implemented a number of precautions, including cleaning and disinfection of all areas. We are also working closely with local and state health officials to stay current on the latest information and recommendations. Please bring your CDC COVID-19 Vaccination Record Card to receive your booster. If you have lost your card or do not have one, please contact the vaccination site where you received your first and second doses and ask how to obtain a new card.
Those over the age of 18 are eligible for a Moderna booster.
If you received Moderna or Pfizer for your original series, you must wait at least six months to receive your booster. If you received Johnson & Johnson, you are eligible after two months.
