Job Description

$2000 Sign-on Bonus for Full-time + $350/mo housing stipend

Cliffview Assisted Living is 28 beds located in Kremmling, CO in beautiful Grand County CO.

Reports to:  Cliffview Administrator 

Description: The PCP/Nurse Aide/QMAP is responsible for providing care to the 28 resident population of Cliffview Assisted Living Center within their Scope of Practice.  They are responsible for the provision of basic nursing care to patients by maintenance of a safe and clean patient care environment. The PCP/Nurse Aide/QMAP participates as an integral member of the health care team to establish and maintain healthy relationships with residents, resident family members, health care team members, and communication of information which contributes to optimal patient care and the achievement of desired and defined patient outcomes.

Requirements:

• High school graduate or GED equivalent.
• Licensed as a Certified Nurse Assistant in the State of Colorado preferred.
• Graduate from a State Certified Program for Nursing Assistants preferred.
• Basic Life Support – BLS (or within 60 days of hire date).
• 6 months prior experience as a CNA preferred.

Compensation: $19.00 – $28.74 per hour DOE plus Nights differential; $2000 Sign-on + FT perm are eligibile for Housing Stipend available for residents of Grand, Summit and Jackson County.

Benefit eligible employees include both full time and part time employees. MPH offers Health, Dental, Vision, Life & Disability, STD and FSA. All employees (excluding temporary) are eligible to participate in our retirement program.

Middle Park Health’s Granby Campus is currently without power due to an area outage. At this time we are awaiting information on when power will be restored. We are running on generator power to essential areas of the hospital like the Emergency Department which will remain open.

Our primary and specialty clinics have preemptively cancelled morning appointments. Our registration and scheduling staff is reaching out to all patients to reschedule. Physical Therapy patients are still being seen as scheduled.
