Bio: Jacob earned his doctorate degree in physical therapy from Concordia University, Wisconsin in 2011.  He serves as a lead physical therapist while covering sports, geriatrics, and general outpatient orthopedics.  When he is not working in the clinic, he loves spending time with his family, enjoying all things outdoors, and being involved in sports on many levels.

Middle Park Health’s Granby Campus is currently without power due to an area outage. At this time we are awaiting information on when power will be restored. We are running on generator power to essential areas of the hospital like the Emergency Department which will remain open.

Our primary and specialty clinics have preemptively cancelled morning appointments. Our registration and scheduling staff is reaching out to all patients to reschedule. Physical Therapy patients are still being seen as scheduled.
