Visit Middle Park Health in Grand Lake for the full spectrum of primary care services, including well-visits and vaccines. Seek care for urgent concerns and receive a referral to specialty or emergency care where appropriate. We also provide lab diagnostics on-site. The health care providers at Grand Lake will review your medical history and assess your risk for acute and chronic illnesses. They perform screening tests as needed and counsel each patient about healthy lifestyle measures that can lower avoidable risk factors. We can also coordinate care throughout the Middle Park Health system for patients with chronic health conditions, including asthma, cancer, diabetes, hypertension, and heart disease.
