Chronic Care Management

CCM in Grand County, Colorado

Middle Park Health is pleased to offer CCM (Chronic Care Management), a program for Medicare patients managing multiple chronic conditions.

Chronic Care Management

Middle Park Health offers expert cardiology care and the latest diagnostic technologies to get a clear picture of your heart health. A variety of non-invasive and minimally invasive tests are available, including:

  • Who Qualifies for CCM?

Any Middle Park Health patient insured by Medicare or a managed Medicare supplement who has two or more chronic health conditions, for example, diabetes and high blood pressure.

  • What is involved in CCM?

A supportive monthly phone call with our caring Integrated Health Nurse to provide personal care that focuses on your goals, develops a comprehensive plan of care, and answers questions you might have.
If your condition changes, you have telephone access to the healthcare team in between Primary Care appointments to help with decision making – supporting you to decide whether to make a clinic appointment or go to the Emergency Room.

  • What do I get out of CCM?

A consistent point of contact at our Primary Care Clinics and access to a coordinated team of dedicated health care professionals who help you stay on track between office visits.

Education and support for living fully with your condition and even improving your health by managing symptoms and pain as you consider small changes that have great positive impact.

Help with identifying anything holding you back and finding answers and resources to actually support you.

Early support to navigate the sometimes complex world of healthcare to prevent escalation of your condition and a prevent a health crisis.

A reliable resource for understanding your medication.

Help to gather clinical information from your Specialists and ensuring your Primary Care Practitioner (PCP) has that information to support your plan of care.

  • What is the goal?

Regular CCM helps you manage your care more effectively, so you can spend more time focusing on your health and your life with a clear plan of care.

You receive a comprehensive care plan that reflects your concerns and health goals. It also gives you the opportunity to work with your PCP to make informed decisions and manage your conditions with greater focus and confidence.

Your care plan also supports you between visits with your PCP and specialists, connects you with outside resources and referrals, and helps guide you through challenges along the way.

  • Is there a cost to me?

This program is covered by Medicare and subject to your plan’s deductible. CCM is funded at 80% after your deductible is met. In general, the out-of-pocket (copay/ coinsurance) cost is around $10-15 / month once the deductible is met. This covers the non-face-to-face services received through the program.

Important Annoucement

Middle Park Health’s Granby Campus is currently without power due to an area outage. At this time we are awaiting information on when power will be restored. We are running on generator power to essential areas of the hospital like the Emergency Department which will remain open.

Our primary and specialty clinics have preemptively cancelled morning appointments. Our registration and scheduling staff is reaching out to all patients to reschedule. Physical Therapy patients are still being seen as scheduled.
